9/14/2023 EnchantaVaughn Approved Behavior: QMHPA greeted Kaila upon arrival to the home setting. Upon arrival to the home setting, Kaila appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her morning. Kaila verbalized to QMHPA that her morning was going good. QMHPA explained to Kaila that this session was going to focus on increasing her independent living skills. Kaila did not present any symptoms of S/I or H/I at the time of the session. Intervention: QMHPA began the session by inquiring about Kaila’s current thoughts, feelings and emotions. QMHPA discussed with Kaila identifying daily needs. QMHPA educated Kaila on appropriate budgeting skills and saving money. QMHPA discussed with Kaila the difference between a need and a want. QMHPA quizzed Kaila on needs and wants. QMHPA explained to Kaila the importance of managing funds successfully. QMHPA educated Kaila on ADL's and ILS's. QMHPA discussed with Kaila the importance of proper grooming and personal hygiene. QMHPA engaged Kaila in a skills building activity. QMHPA explained to Kaila bathing, wearing clean clothes daily and completing task. QMHPA processed with Kaila how good hygiene will increase independent skills. Response: Brown appeared to be receptive to QMHPA’s interventions and services that were provided to her during the session. During the session, QMHPA determined that Kaila will get her needs and wants mixed up. Kaila has trouble with differentiating needs and wants. Kaila said she need to buy food outside the home when she has food in the home to cook. Kaila said she saves her money but will spend it. Kaila said if she holds her money stated clothing, food and a house is a need when quizzed. Kaila was agreeable to personal hygiene regiments. Kaila stated she needed some deodorant and shampoo. Towards the end of the session Kaila discussed with QMHPA that she has to maintain her household for everyone else in her house and that is becoming depressing as well. Plan: QMHPA will examine the problematic IADL areas with the client to identify any patterns of behavior or cognitions that cause the failure at independent functioning (1-3x weekly).
